All Medipouch are available in less than 23” waist size. 
Simply place your order for any compact or standard pouch.
During checkout you will find a section for special instructions,
in this section note the current waist size of the person who will be wearing the pouch.
I will make the belt to fit now with room to grow.

Waist size over 42” 

Medipouch is available in waist sizes over 42” in the solid black color only.
